An acceptable emergency in which to use a building business credit card would include a medical or dental situation where payment is required upfront, a breakdown of their (not their friends’) automobile, being stranded at a party where the driver is drunk and your child has no way home, being otherwise stranded somewhere with no way home, being burned out of an apartment and needing to pay for a place to stay or to buy food (while out of an apartment). These may sound very ominous, but that should be the tone around the use and abuse of a business line of credit card.

The biomechanics of wheelchair propulsion in individuals with and without upper-limb impairment.
We used an instrumented medicare power wheelchairs ergometer and 3D motion analysis system to collect joint kinematic and temporal data, as well as hand rim and joint kinetics, in 47 manual power wheelchairs users (MWCUs) (15 with upper-limb impairment and 32 without upper-limb impairment). The group with upper-limb impairment propelled with a higher stroke frequency and reduced hand-rim contact time, and smaller peak joint angles and joint excursion of the wrist, elbow, and shoulder during the contact phase. They also propelled with a reduced power output and reduced hand-rim propulsive and resultant forces, moments, and joint compressive forces. We concluded that these kinematic and kinetic strategies might be a mechanism for allowing MWCUs with upper-limb impairment to remain independent. Additionally, the reduced joint excursion and reduced magnitude of forces may protect them from the development of secondary upper-limb pathologies.

Influence of hand cycling on physical capacity in the rehabilitation of persons with a spinal cord injury: a longitudinal cohort study
OBJECTIVE: To investigate the influence of handicap scooters on outcome measures of physical capacity during and after rehabilitation in persons with paraplegia and tetraplegia in The Netherlands. DESIGN: A longitudinal cohort study with measurement moments at the start (t1) and end (t2) of clinical rehabilitation and 1 year after discharge (t3). Hand cycle use was assessed by means of questionnaires at t2 and t3. SETTING: Eight rehabilitation centers in The Netherlands. PARTICIPANTS: Subjects (N=162) with a recent spinal cord injury. INTERVENTIONS: All subjects followed the regular rehabilitation program. MAIN OUTCOME MEASURES: Peak oxygen uptake (Vo(2)peak) and peak power output (POpeak) determined in a handrim medical scooters peak exercise test, peak muscle strength of the upper extremities, and pulmonary function. RESULTS: A significantly larger increment in Vo(2)peak, POpeak, and elbow extension strength was found in subjects with paraplegia during clinical rehabilitation. No such effect was found in subjects with tetraplegia. In the postrehabilitation period, no influence of hand cycling on any outcome measure was found in subjects with paraplegia or subjects with tetraplegia. CONCLUSIONS: After correction for baseline values and confounders, regular hand cycling (once a week or more) appeared to be beneficial for improving aerobic physical capacity in persons with paraplegia during clinical rehabilitation. The small and heterogeneous study groups may have hampered the finding of positive results of hand cycling in persons with tetraplegia.

Consequences of a cross slope on wheelchair handrim biomechanics
OBJECTIVE: To test the hypothesis that pushing on a cross slope leads to increased handrim loading compared with that found on a level surface. DESIGN: Case series. SETTING: Biomechanics laboratory. PARTICIPANTS: Twenty-six manual wheelchair users. INTERVENTION: Subjects pushed their own wheelchairs on a research treadmill set to level, 3 degrees , and 6 degrees cross slopes. Propulsion speed was self-selected for each cross-slope condition. Handrim biomechanics were measured for the downhill wheel, using an instrumented power wheelchairs wheel and a motion capture system. MAIN OUTCOME MEASURES: Speed, peak kinetics (force, rate of loading, torque), push angle, cadence, push distance, and medicare power wheelchairs power output were averaged over a 20-push set for each subject and each cross-slope condition. Outcomes were compared across cross slopes using a repeated-measures analysis of variance. RESULTS: Push angle and cadence were unaffected by cross slope. A trend of decreasing self-selected speeds with increasing cross slope was not significant. There were considerable increases in the peak kinetic measures, with the axial moment increasing by a factor of 1.8 on the 6 degrees cross slope (P=.000). More pushes were required to cover the same distance when on a cross slope (P<.034). The power required for propulsion increased by a factor of 2.3 on the 6 degrees cross slope (P=.000). CONCLUSIONS: Users must push harder when on a cross slope. This increased loading is borne by the users' arms, which are at risk for overuse injuries. Exposure to biomechanic loading can be reduced by avoiding cross slopes when possible.

Disability type influences heart rate response during power wheelchair sport
PURPOSE: The purpose of this study was to determine the influence of disability type on exercise response during power wheelchairs and handicap scooters competition. The secondary purpose was to determine the extent to which heart rate responses during competition meet cardiorespiratory fitness training intensities for the general population. METHODS: Forty-eight athletes who had cerebral palsy (CP, N = 31), spinal cord injury (SCI, N = 10), or muscular dystrophy (MD, N = 7), and were competing in the 2003 Power Soccer National Tournament, volunteered to participate. Heart rate was recorded every 5 s throughout pre-game and game conditions by Polar S610 monitors. Average heart rate (HR) values were determined for GAME and RESPONSE (change score between GAME HR and pre-game HR). The Kruskal-Wallis nonparametric test was used to determine whether a significant difference among group medians existed on the dependent measure, RESPONSE (P < 0.05). RESULTS: A significant difference on RESPONSE (P < 0.05) existed among athletes with CP (29 bpm), SCI (17 bpm), and MD (26 bpm). The median RESPONSE for athletes with CP was 12 bpm higher than athletes with SCI, and this difference was significant (P < 0.01). Further, 22 athletes with CP (71%), 5 athletes with MD (71%), and 1 athlete with SCI (10%) exceeded 55% of estimated HR(max) for at least 30 min during competition. CONCLUSION: Disability type influences the heart rate response to power wheelchair sport, and may affect the ability to sustain training intensities associated with fitness improvement.
